This tool is designed to facilitate the rational conversion of one opioid regimen to an approximately equianalgesic dose of another. The dosages and conversion ratios are based specifically on the Asian patient profile, and common local practices. We have taken every measure to harmonize the ratios across local institutions. However, we recognize that usage can vary from case to case, and institutional practices might still vary. Users of this program should be limited to practising clinicians who prescribe or advise on prescribing opioids. As with any clinical tool, we strongly encourage clinicians to use the information provided within the context of each individual patient.
