The Serious Illness Conversation Guide was first developed by Ariadne Labs, a joint center for health systems innovation at Brigham and Women's Hospital and the Harvard T.H. Chan School of Public Health. LCPC has adapted it for our local population through iterative patient testing. This resource is free for use by all healthcare professionals.

• This workshop is pending accreditation from the Singapore Medical Council, Singapore Nursing Board & Singapore Pharmacy Council.
• Continuing education credits are available for LCPC events through the Singapore Medical Council (SMC), Singapore Nursing Board (SNB), Singapore Pharmacy Council (SPC) and Allied Health Professions Council (AHPC), subject to their respective approvals.  
  

  Upon completion of the workshop, participants can receive up to 6 CPE points from SMC/SNB/AHPC or up to 5 CPE points from SPC. 

  Attendees who wish to earn credits are required to sign the relevant registration form along with their CPE/CME number on the day(s) of the events. 

  The attendee’s registration details will be submitted after the event and credited upon receiving respective approvals. 

• Workshop participants will be awarded with an e-Certificate of Completion at the end of the workshop*.

*The e-Certificate of Completion will only be awarded to participants who achieved at least 80% attendance for face-to-face teachings and completion of all coursework (including assessments and evaluation)
