The SG Pall eBook includes an opioid conversion tool. This tool is designed to facilitate the rational conversion of one opioid regimen to an approximately equi-analgesic dose of another. The authors have strived to ensure that the information included in this programme reflects the current level of knowledge regarding opioid conversions and it must be noted that the dosages and conversion ratios in this Opioid Conversion Calculator are based specifically on the Singaporean patient profile and common local practices. In Singapore, we have a multicultural society with 4 major ethnic groups; Chinese, Malays, Indians and Eurasians. While we have taken every measure to harmonize the ratios across local institutions, the user must recognize that usage can vary from case to case due to varying institutional practices, and opioid conversion being subject to inter-individual variability.

Users of the Opioid conversion calculator should be limited to practicing health care professionals who prescribe or advice on prescribing opioids. As with any clinical tool, health care professionals must use the information provided within the context of each individual patient. In case of conflict or doubt, please consult a palliative care specialist for further advice.
