Cater to patients diagnosed with serious illness(es) and have dedicated caregivers at home. Home visits are nurse-led, and supported by doctors, counsellors, social workers and therapists. Frequency of visits depends on clinical needs. Caregiving is done by patient’s caregivers, not the home care team.

Facilities
- Patients suffering from serious illness(es) with estimated prognosis of less than 1 year.
- Majority of the home hospice providers are free of charge to patient. Government subsidies claimable by the service based on means testing.
