The Lien Centre for Palliative Care (LCPC) is pleased to host an
electronic palliative care handbook, SG Pall eBook, with the objective
of making basic palliative care information and
knowledge available to the health care professionals in Singapore.
LCPC’s educational mission is to deliver high quality, palliative care
education at the generalist level to meet the growing palliative care
service needs locally and regionally.
This eBook has been created by practicing palliative care doctors and
nurses in Singapore. The content is most appropriate for doctors,
nurses, and allied health care professionals who encounter patients with
palliative care needs in their practice.
The information presented in this SG Pall eBook is correct at point
of publishing and is made available in good faith, derived from sources
believed to be reliable and accurate at the time of release for
educational purposes only. While we do our best to keep the information
up-to-date and accurate, with rapid advances in the medical field, new
information might be available that is not reflected here. As such, LCPC
does not purport information on this website be complete or updated at
all times.
The SG Pall eBook includes an opioid conversion tool. This tool is
designed to facilitate the rational conversion of one opioid regimen to
an approximately equi-analgesic dose of another. The authors have
strived to ensure that the information included in this programme
reflects the current level of knowledge regarding opioid conversions and
it must be noted that the dosages and conversion ratios in this Opioid
Conversion Calculator are based specifically on the Singaporean patient profile
and common local practices. In Singapore, we have a multicultural
society with 4 major ethnic groups; Chinese, Malays, Indians and
Eurasians. While we have taken every measure to harmonize the ratios
across local institutions, the user must recognize that usage can vary
from case to case due to varying institutional practices, and opioid
conversion being subject to inter-individual variability.
Users of the Opioid conversion calculator should be limited to practicing health care professionals
who prescribe or advice on prescribing opioids. As with any clinical
tool, health care professionals must use the information provided within
the context of each individual patient. In case of conflict or doubt, please consult a palliative care specialist for further advice. It is the user's responsibility
to integrate the information from the opioid calculator with clinical
knowledge of his or her patient and after review of product information
of the medication. In no circumstance should any
information in this opioid calculator be deemed, relied on or
interpreted as medical advice, diagnosis or treatment in relation to the
treatment of a patient. Any advice, diagnosis or treatment for any
particular patient and the prescription of opioid dose remains the
ultimate responsibility of the prescribing physician.
LCPC and the development team are not responsible for how
providers use the information in the SG Pall eBook (including the use of
the Opioid Conversion Calculator).
LCPC specifically disclaims all warranties with respect to this SG
Pall eBook and any of its contents, whether expressed or implied,
